GOALS:
Record answers on the Continence Assessment Nurses' Outcome Record (CANOR) each time the Continence Assessment Nurses' Guide is used. Enter the number on CANOR which most accurately describes the patient's usual patterns. When two numbers are used to describe usual patterns, record the numerical average of the numbers. It is suggested that CANOR be hole-punched for placement in the patient's chart.
CANOR was designed to track continence progress (or lack thereof) over time. Blank lines are included for individualization. This consistent scoring mechanism could be used at each following-up visit related to continence. It is a valuable measurement tool to help evaluate patient treatment outcomes.
