Directions: Answers may be recorded either on CAN-Guide (circle responses) or on the separate Continence Assessment Nurses' Outcome Record (CANOR). Use only the items relevant to the patient's condition. The scale is ranked 1-5, with 5 being the "best" or "most favorable" function. Circle the number on CAN-Guide or enter the number on CANOR which most accurately describes the patient's usual patterns. Sometimes two numbers may be circled or entered and their average will be recorded. CAN-Guide may be hole-punched to put directly into a patient's chart. CANOR was designed to be used for scoring at follow-up visits. It is a valuable tool to do serial tracking of patient treatment outcomes.
