This boy had a straddle injury. He fell on "monkey bars" and had swelling and ecchymoses on his perineum and blood from his urethral meatus. He came to the Emergency Room here at Children's Hospital in urinary retention. A suprapubic tube was placed percutaneously and a voiding cystourethrogram was attempted. He was unable to void and so a retrograde urethrogram was done. It showed a small area of extravasation and urethral spasm just cephalad to this (Figure 1). The urethra was intact. The study was performed on May 26. He was kept on suprapubic drainage. On June 15 a second retrograde urethrogram was done and it showed a large pseudodiverticulum containing clot (Figure 2). The third study was a voiding cystourethrogram (he was finally able to void) and it showed a normal urethra (Figure 3). He will be followed closely for the development of a stricture.
