PMH/PSH: Significant for recurrent urinary tract infections with one episode of right pyelonephritis.

Physical Examination: Unremarkable.

Laboratory Values:

Urine culture: > 100,000 E. coli

Creatinine: 1.0 ml/dl

WBC: 8,500 mm³
